VAN WERT — The following individuals appeared Monday before Judge Martin Burchfield in Van Wert County Common Pleas Court:

Arraignment

Aaron Crutchfield, 36, Van Wert, entered a not guilty plea to a charge of domestic violence, a felony 3. He was released on a surety bond with an order to have no contact with the victim. Pretrial set for Nov. 8.

Bond violations

Chad Fisher, 41, Van Wert, admitted violating his bond by failing to report. He was re-released on surety bond with a twice weekly reporting requirement. Pretrial set for Nov. 8.

Kessa Eddins, 33, Van Wert, admitted violating her bond by failing to report. Her new bond was set at $10,000 cash or commercial surety. Pretrial set for Nov. 8.

Time waiver



Donald James Smith, 39, Van Wert, signed a time waiver in open court and requested additional time to prepare his case. Pretrial scheduled for Nov. 29.

Changes of pleas

Chad Fisher, 41, Van Wert, changed his plea to guilty to possession of cocaine, a felony 5. A second charge of possession of heroin, a felony 5, was dismissed for his plea. The court ordered a pre-sentence investigation and set sentencing for Nov. 29.

Lisa Dull, 54, Van Wert, changed her plea to guilty to aggravated possession of drugs, a felony 5. The court ordered a pre-sentencing investigation and set sentencing for Nov. 29. She was released on a surety bond with condition that she report to probation at least three times per week.

Ricky Welker, 35, Van Wert, entered pleas of guilty in two different cases. In the first case, he pleaded guilty to two separate counts of assault on police officers, each a felony 4. A misdemeanor charge of resisting arrest in that case was dismissed for his pleas. In the second case, he pleaded to domestic violence, a misdemeanor 1, reduced from a felony 4. The Court ordered a pre-sentence investigation and set sentencing for Nov. 29.

Violations

Paul Bakle, 34, Van Wert, appeared for a probation violation, but hearing was continued until Nov. 8.

Jacquelyn Forthman, 28, Ohio City, admitted violating her probation by testing positive for drugs. She was sentenced to 180 days jail in each of two cases, concurrent, credit for 158 days in one case and 160 days in second case, leaving 22 days to serve.

Andrew Thomas, 36, Ohio City, denied violating his probation by testing positive for drugs. Hearing set for Nov. 15. Request for surety bond denied and bond set at $100,000 cash or commercial surety.

Sentencing

Nicole Cooper, 35, Van Wert, sentenced after previously admitted to violating her probation by testing positive for drugs. She was sentenced to: five years community control, up to six months at WORTH Center, 200 hours community service, 30 days jail at later date, substance abuse assessment and treatment, three years intensive probation and was ordered to pay partial appointed counsel fees, monthly probation fee and court costs.

Samantha Lacy, 25, Paulding, aggravated possession of drugs, a felony 5. She was sentenced to: five years community control, up to six months WORTH Center, 60 days jail at later date, 50 hours community service, to seek employment, obtain GED, substance abuse assessment and treatment, three years intensive probation and was ordered to pay court costs, monthly probation fee and partial appointed counsel fees.
